Training Permission / Opt Out Form

The parish may choose either of the following options for determining which minors in their programs will not participate in annual Safe Environment Training:

At time of registration:

~Permission Form~

Have parent/guardian complete this permission form for every child enrolled in program, indicating that they may / may not receive training.

OR

~Opt-Out Form~

Have parent/guardian complete this form only if he/she dose not want child to receive training.
